How to Schedule Your Visits Out:

Wondering how many visits you have left or what the recommended frequency was for your adjustments? You can always email, text or stop by the front desk at your next appointment to request this information! We also have Customized Scheduling Tools available to assist you in the the process.

How to Enter:

After you have scheduled out the entirety of your care plan by email or text, input your patient information below and submit, it's that simple!

All entrants will be verified.

Reactivate Your Care at Revolution:

If you are a previous patient at Revolution Wellness Center but have not been seen in the last 12 months, you are still eligible to enter!

1. Call or Text 763-425-4577 or send an email to [email protected] to request a Reactivation Exam

2. Receive care plan recommendations based on your exam

3. Schedule your visits out accordingly

4. Enter your patient information below!
